Pregnancy and Parental Leave “To Do” List (Updated Sept 2014)
Here is the procedure to follow:
Review the Collective Agreement Articles 17 and 18 regarding Pregnancy and Extended Leaves.
Obtain a medical certificate stating the expected date of birth.
Fill in a Request for leave of Absence (LOA) form stating the FIRST day of leave and the RETURN to work date (first day must be on or before your due date), get it signed by your Principal.
Forward the completed LOA form and medical certificate to Shannon Mc Fadden, Human Resources, Chatham Office.
If your baby arrives earlier than the scheduled commencement of your Pregnancy Leave, you will need to contact Human Resources in order to revise your start date.
Your ROE will be sent electronically to HRDC (EI) by the Payroll Dept. as soon as practical following your last day of work.
Complete an application for EI Benefits (Service Canada 1-888-534-2222 or www.servicecanada.gc.ca). This can be done in advance of your ROE being issued.
Send a copy of “My Current Claim” and “My Payments” pages from your Service Canada account website to the Human Resources Office at the Sarnia Education Centre. This is required to receive your 8 weeks of Pregnancy Leave Benefits Payments.TOTAL: 8 weeks of 100% pay (full and or supplementing your EI benefit) through the course of the entire calendar year during instructional days AND holidays.
HR will provide you with details regarding your benefits, pension, etc, while on Leave.
Review Information about health benefits (www.greatwestlife.com or 1-800-957-9777 Division 1 Group 51804), pension contributions (OTPP or 1-800-668-0105) and LTD (OTIP user name: L7010-901-A1 password 5684548 or 1-877-260-3892). Contact the Benefits Officer (Susan ). ETFO now has a MANDATORY LTD PLAN while you are on leave.
Add your child to your benefit plan as soon as possible after the birth (within 30 days)
Pay your OCT fee in January ($150) www.oct.ca , this is due by April 15th
